Ongoing support · California

A clearer plan. A team to help you follow through.

Bone-health appointments, medication questions, and changes in mobility can be hard to keep track of between visits. Bonegevity helps you organize the next step—and follows what is still waiting to happen.

You or a caregiver can reach out directly. You do not need a recent hospital stay to ask about ongoing support. Clinical care is available in California, subject to fit and availability.

Support you can use

Know the plan. Know the next step.

A living care plan brings bone health, muscle, and mobility together. Your team keeps track of open tasks, helps organize questions, and brings decisions back to the appropriate clinician.

A clearer picture

Make sense of your current plan, testing, medications, and the follow-up that still needs attention.

Human follow-through

Work with a care team that tracks what is pending, completed, or ready for a clinician’s decision.

Support that fits

Choose a level of ongoing contact with the team. Involve a caregiver with your permission.

Help keeping care organized

Eligible people with qualifying long-term conditions may receive Chronic Care Management (CCM). The team reviews eligibility and expected costs before enrollment.

Original Medicare alone does not establish eligibility. Deductible and coinsurance may apply.

We review clinical fit, coverage, and expected costs before enrollment. Read the details of each pathway below.

Original Medicare · Fee-for-service

Chronic Care Management

For people with qualifying long-term health conditions who want help keeping their care organized between visits. CCM can include a shared care plan, medication review, coordination with other providers, and support after a hospital stay.

  • Medicare Part B may cover CCM for people with two or more qualifying chronic conditions expected to last at least a year or for the rest of their life.
  • The team reviews clinical criteria, benefits, and any care management you already receive. Original Medicare alone does not establish eligibility.
  • Part B deductible and coinsurance may apply. We explain expected costs and obtain consent before starting CCM; an initial visit may be needed.

Medicare Advantage and HMO participation is not currently available through this pathway. Ask us to review your options if you are unsure of your plan.

Private pay · Two levels of support

A subscription for ongoing follow-through

For people seeking a private-pay option, with more hands-on support through Guided Continuity or a lighter cadence through Monitor.

  • Choose the level of contact and coordination that fits your needs.
  • See the included services and monthly or annual price before requesting a conversation.
  • Review the plan, billing terms, and start date with the team before you enroll or pay.

You do not need to buy a subscription to receive appropriate Medicare-covered care. A subscription is an optional private-pay service, not insurance. We explain what insurance may cover and what, if anything, you would pay separately.

The right amount of support for you.

The main difference is how much scheduled contact and coordination you want. Guided Continuity adds a monthly conversation and broader follow-up support. Monitor uses monthly messages and a more detailed review every three months.

More hands-on support

Guided Continuity

$179 / month

Or $1,790 per year, billed annually

A good fit if: you or a loved one would like a scheduled conversation each month and more help keeping appointments, testing, and other next steps moving.

  • Your monthly contact: a scheduled check-in to talk through your care plan, questions, and what still needs attention.
  • Between check-ins: the team follows up on agreed tasks and keeps your clinician’s instructions and next steps organized as needs change.
  • Help with follow-up: broader coordination around medications, bone-density scans (DEXA), and lab testing. Your clinician makes treatment decisions.
  • Additional support: caregiver involvement with your permission, appropriate strength and mobility checks, and help organizing questions for your treating clinicians.

What the higher price adds: a scheduled monthly conversation and broader coordination. A device or remote-monitoring program is not included in this price.

Request Guided Continuity

Lighter ongoing support

Monitor

$99 / month

Or $990 per year, billed annually

A good fit if: you feel comfortable acting on your care plan and would like regular messages and help keeping the next steps visible.

  • Your monthly contact: a message-based check-in you can answer without a scheduled call.
  • Between check-ins: the team tracks the agreed next steps, with a more detailed review of your needs and plan every three months.
  • Help with follow-up: focused tracking of agreed tasks. The scheduled monthly conversation and broader coordination offered in Guided Continuity are not included.
  • If your needs change: we review whether more support would help and discuss your options, including Guided Continuity or eligible insurance-supported care.

What the lower price includes: regular messages and periodic review, with a lighter level of coordination. A device or remote-monitoring program is not included in this price.

Request Monitor

Important information before enrollment

Coordination and medical decisions: Both subscriptions support your existing care plan and its follow-through. Diagnoses, prescribing, and treatment changes remain with your treating physician or other authorized prescribing clinician.

What costs extra? Medications, lab tests, bone-density scans, and devices are not included unless separately agreed in writing. Some may be covered by insurance. We discuss separate services and expected costs with you before you agree to them.

If you qualify for Medicare CCM: care-plan support and coordination may be part of your covered care. You do not need to add Monitor or Guided Continuity to receive those covered services, and we do not charge you a subscription fee for the same work billed to insurance.

Before paid enrollment, we confirm your included services, start date, payment timing, and renewal, cancellation and refund terms in writing. Requesting a conversation does not enroll you, start billing, or authorize a payment.

How do you follow changes over time?

Tracking changes in strength, balance, and mobility over time—so patients and their care teams know when follow-up may be needed. The care team selects appropriate checks and reviews available results alongside your history and care plan. Technology supports that review; it does not replace clinical judgment.

Please call (949) 619-5520 to discuss ongoing support if this form is unavailable.

Can a caregiver help me get started?

Yes. A caregiver can request a conversation and help with the next steps. We confirm the patient’s permission before sharing care information or involving someone in care.

What if I already receive Medicare care management?

Tell us who currently helps manage your care. We will review those services with you before recommending a plan. You do not need a subscription to receive appropriate covered CCM care, and we do not charge a subscription fee for the same work billed to insurance. Asking about Bonegevity does not transfer your care or authorize billing.

Does “Monitor” mean emergency monitoring?

No. Monitor is the name of the lighter subscription, with a monthly message-based check-in and quarterly review. Neither subscription provides continuous monitoring, emergency response, or unlimited physician access. Call 911 for an emergency.

Do I need an app or wearable?

You do not need to download an app or buy a wearable to request support. Before enrollment, we will discuss a practical way for you to receive messages and share information. Devices are not included in these prices, and technology supports the human care team.

Our purpose

Supporting safer, more independent living at home.

Bonegevity works alongside your existing clinicians. It does not replace their care and is not an emergency service.
