Questions about Bonegevity?
Clear answers about getting started, eligibility, costs, and ongoing support.
Getting started
Who is eligible for Bonegevity?
Bonegevity provides clinical care in California for recovery and ongoing bone and musculoskeletal needs. Options include services for eligible Original Medicare patients and separate private-pay subscriptions. The team reviews clinical fit, program criteria, current services, and availability before enrollment.
Do I need a referral to get started?
You may submit an intake yourself or with help from a caregiver. Referral requirements can vary by service and coverage, so our team will review the appropriate next step. Case managers and healthcare professionals may also refer patients directly.
Can a family member reach out?
Yes. Many requests come from adult children, spouses, or friends helping someone recover at home. A caregiver may complete the intake even if they do not know every detail yet.
What happens after I submit my information?
Our clinical team reviews the intake, assesses clinical fit and applicable eligibility requirements, and contacts the patient or caregiver with the appropriate next steps.
What should I expect during the first call?
A team member will review your request, ask which next steps are difficult to manage, and explain how Bonegevity can help organize follow-through. You do not need to have every record or answer ready.
How quickly can someone get started?
The team reviews clinical fit, eligibility, and available services before confirming next steps. Submitting an intake does not guarantee a start date.
Cost & coverage
Is Bonegevity covered by Medicare?
Medicare Part B may cover Chronic Care Management for eligible people with qualifying chronic conditions. This can include care-plan support, medication review and coordination with other providers. Deductible and coinsurance may apply. You do not need to buy a subscription for covered CCM services, and we do not charge a subscription fee for the same work billed to insurance. Medicare Advantage and HMO participation is not currently available through this pathway.
Will I have any out-of-pocket costs?
For covered services, deductible and coinsurance may apply. Private-pay Guided Continuity costs $179/month or $1,790/year; Monitor costs $99/month or $990/year. Annual options are billed annually. Medications, lab tests, bone-density scans and devices are not included unless agreed in writing; some may be covered separately by insurance. We explain services, expected costs and terms before enrollment. A website request does not enroll you or start billing.
Do you offer ongoing care?
Yes. Patients and caregivers can reach out directly for ongoing bone, muscle, and mobility support. Guided Continuity includes a scheduled monthly conversation and broader coordination. Monitor offers monthly message-based check-ins and a deeper review every three months. Eligible Original Medicare patients can ask about Chronic Care Management (CCM). You do not need a recent hospital stay to request support.
What Bonegevity does
What is closed-loop bone health?
It means the next step stays visible. Bonegevity helps organize records, appointments, medication questions, and other care needs, tracks what is pending or completed, and brings questions to the appropriate clinician.
Do you only help orthopedic patients?
No. We have a strong musculoskeletal and bone health focus, but we also help with broader post-discharge recovery and coordination when those needs affect safety, mobility, and follow-through.
Is Bonegevity only for after discharge?
No. Bonegevity also helps organize ongoing osteoporosis care, medication questions, bone-health testing, and strength and mobility needs between visits.
How are changes in strength and mobility followed?
Tracking changes in strength, balance, and mobility over time—so patients and their care teams know when follow-up may be needed. The care team reviews available measurements and check-ins alongside your history and current plan. Technology supports that review; it does not make clinical decisions.
What kinds of strength and mobility checks might be used?
Depending on your situation and safety, the team may review walking speed, chair stands, balance, grip strength, falls, or near-falls. Your care team selects appropriate checks; they do not replace a physician's diagnosis.
Technology & your care team
Do I need an app or wearable to receive support?
No. Patients can take part through phone-based check-ins and clinical measurements. Your care team will explain which options fit your situation.
Does Bonegevity replace my primary care provider or orthopedic surgeon?
No. Bonegevity adds an additional layer of support and works alongside primary care, orthopedic care, home health, therapy, and other treating professionals. Your physicians continue to diagnose, prescribe, and make treatment decisions.
