About this policy

This policy explains information handling for Bonegevity’s public website and online forms. Information collected as part of clinical care is also subject to the applicable clinical privacy notices and requirements. This website policy is separate from the Notice of Privacy Practices.

Information you provide

Contact forms ask for your name, contact details and message. Intake and referral forms may also request patient demographics, caregiver or referring-party details, insurance information, health concerns and supporting documents. Submit information only for yourself or someone you are authorized to assist. Use the intended intake or referral pathway for clinical information rather than the general contact form.

How information is used

Bonegevity uses submitted information to respond to questions, contact patients and referring parties, assess requested services, coordinate intake and care, and maintain records of communications. General contact submissions are stored in the application and routed to referrals@bonegevity.net. Patient and referral submissions may be associated with a patient chart.

Website operation and service providers

Hosting, storage and communication providers help operate the website and related services. Access and sharing depend on the purpose of the request and the services provided.

Cookies and technical information

The website and supporting infrastructure may process technical information needed to deliver pages, protect services and troubleshoot errors. Staff sign-in uses session cookies in the application.

Retention and privacy requests

Information retention depends on its purpose and applicable recordkeeping requirements. Contact Bonegevity to ask about access, correction, privacy choices or a concern. Some clinical records may need to be retained even when services end.

Contact and updates

Call (949) 619-5520 or use the Contact page to request the appropriate privacy contact. Avoid sending detailed medical records through a general privacy inquiry. Bonegevity may update this policy as practices change; the effective date will identify the published version.
