Patient intake

Start with the basics. Phone or email is enough to begin.

Address, caregiver, primary care, and pharmacy
This first step should take less than a minute.
Have you had an inpatient, observation, rehab, or skilled nursing stay in the last 30 days?
What kind of help do you need most right now?
Has anyone from your care team contacted you since discharge?
Do you have a follow-up visit scheduled?
Have your medications changed recently?
Secondary or supplemental insurance

If a card image is needed, the care team will request it through a secure channel after intake.

Do you have two or more long-term health conditions that need ongoing care?
Do any of these apply to you?
Would ongoing monthly support be helpful?

This intake helps Bonegevity identify likely recovery support, ongoing care coordination, and private advocacy needs without asking you to understand any billing language up front.
